INSURANCE

Susan Underwood Physical Therapy is in-network with Blue Cross Blue Shield and Medicare.
If you are a member of a Blue Cross Blue Shield PPO (Preferred Provider Organization) or Medicare,
it is your responsibility to:

Provide us with your insurance information. This information is requested on the Patient Information form, which we ask you to complete before your initial appointment.

Pay your deductible, co-insurance or co-payment at the time of service.

Pay for any services not covered by your insurance carrier.

Provide a current doctor’s order or prescription if your insurance carrier requires one (i.e. Blue Cross Blue Shield Blue Advantage Plan).
Insurance claims with Blue Cross Blue Shield and Medicare carriers are filed as a courtesy at no charge to you.

Patients insured by carriers other than Blue Cross Blue Shield and Medicare will be considered private pay patients. Claim forms, along with a detailed statement of charges, will be provided upon request so that you may file directly with your insurance carrier for reimbursement.

Insurance carrier information and member number must be provided to us in order to complete claim forms.

We recommend that you check with your insurance company prior to your first appointment in order to determine if out-of-network outpatient physical therapy charges will be reimbursed to you.
Blue Cross Blue Shield Insurance

Charges at time of service are determined based on the type of policy you hold. You may have a co-pay or a deductible to meet.
Medicare Insurance

A deductible of $233 and 20% coinsurance, which may be covered by a secondary insurance policy. The 2020 limit for a beneficiary is $2150. Deductible and coinsurance amounts apply to the therapy cap.
Non-contracted Insurance or Private Pay

$200 (maximum daily charge)
